• Required for a minor who drives him/herself, possibly with younger siblings, to be able to sign out themselves.
  • Parental Sign-Out Waiver Form
• Required to be filled out by parents/guardians who wish to allow their children to travel to and from Holiday Spectacular, Inc. functions with other adult designated drivers.
  • Parental Alternate Driver Permission Form
